Comprehensive Foot Exam

Dr. Cortez will conduct a thorough examination of your feet, ankles, and lower legs to diagnose and treat your foot ailment. We can bring the latest diagnostic tools and techniques directly to your home to ensure accurate diagnosis and treatment if necessary. Referrals to a specialist can be made as needed, 

Diabetic Foot Care

Specialized foot care for patients with diabetes is essential, and a preliminary diabetic foot exam is recommended yearly. Dr. Cortez can work with your primary care physician to manage your foot health and prevent complications. Unfortunately, foot problems are a common result of diabetes. Over time, uncontrolled diabetes can lead to nerve damage, known as diabetic neuropathy, which can cause tingling, painful sensations, and loss of feeling in your feet. This loss of sensation can be incredibly dangerous, particularly if you can no longer detect a callus or cut on your foot. Such issues can lead to wounds, infections, and serious complications. To maintain your foot health, consider incorporating daily home foot care routines. Keep the feet you were born with in good condition.

Ingrown Toenail Treatment

Our goal is to ensure that seniors receive the attention they deserve without the hassle of travel. With over 30 years of experience, I understand the unique needs of elderly patients and strive to offer compassionate and effective podiatry services. Additionally, we provide hospice care and accept referrals from various hospice care providers. 

New patient information

Initial Visit

During the initial visit we will collect essential information such as your medical history, medications, and allergies. This time also allows you to get to know your doctor, ask questions, and understand medical advice. 

To complete your initial paperwork, please prepare the following:

  • Please provide the patient's insurance card 
  •  A list of current prescriptions and/or over-the-counter medication, including dose and frequency. 
  • Additionally, include information about the patient's medical and surgical history. 
  • As well as any recent lab results, x-rays, or relevant records. 

Insurance and Payment Information

You are responsible for co-payments, deductibles, or charges that are not covered by your insurance, including those related to our home foot care services. If you have questions regarding billing or which insurance plans we accept for our house call podiatrist visits, please call our office. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company. Additionally, all prior authorizations for senior podiatry services are your responsibility.
